New Patients

During your first visit to Rocky Mountain Foot and Ankle Center, you will be greeted by out staff. Occasionally, patients believe that since we "just" treat feet and ankles, we don't require any medical information. Not true! Many of our patients require from us prescriptions, surgery, or minor procedures, so it's critical you provide us with your primary physician's name, your list of medical conditions, and your medication and allergy list. Feel free to save time at your first visit by printing out and completing the patient forms in advance of your appointment.

To minimize health care costs and prevent duplication of effort, please bring in all xrays (reports are helpful but not sufficient), MRI or CT scan results, lab results, and make a list of all treatment that's previously been tried, either by you at home or by another physician. If you've been treated for your condition prior to seeing us, please bring with you a copy of medical treatment records.

Please bring a valid insurance card, photo identification, a referral from your primary physician (if required by your insurance), and your copayment or plan to pay in full if you are uninsured or planning a cosmetic procedure.
